Background and Aim: The current generation of adolescents is facing increasing behavioral and psychological problems that require extensive clinical attention. One of the destructive behaviors that adolescents commit due to internal and external stressors is self-harm. Self-harm is defined as the intentional and self-inflicted destruction of body tissue for socially unacceptable purposes. This includes such behaviors as cutting, burning, biting, and scratching the skin. In addition, adolescents with self-injurious behaviors often report high rumination. On the other hand, adolescents with self-injurious behaviors usually face numerous social and psychological challenges. One of these challenges is a sense of inferiority, as these individuals are often judged and rejected by their peers and society because of their self-injurious behaviors. Accordingly, this study investigated the effectiveness of dialectical behavior therapy on rumination and feelings of inferiority among adolescent girls with self-harming behaviors.
Materials and Methods: In terms of its applied purpose, this quasi-experimental study used a pretest/posttest design with a control group and a two-month follow-up period. The study population consisted of adolescent girls with self-harming behaviors referred to educational counseling centers in Isfahan during the academic year 2024-25. In total, 35 adolescent girls with self-harming behaviors were selected using a purposive non-random sampling method and randomly assigned to experimental (n=18) and control groups (n=17). The adolescents in the experimental group received dialectical behavior therapy over 10 weeks in ten 90-minute sessions. Self-Harm Behaviors Questionnaire (SHBQ), Rumination Questionnaire (RQ), and Inferiority Feeling Questionnaire (IFQ) were used in the present study. The data were analyzed using a mixed ANOVA in SPSS software (version 23).
Results: The results showed that dialectical behavior therapy has a significant effect on rumination (F=56.90; Eta=0.63; P<0.001) and inferiority feeling (F=40.49; Eta=0.55; P<0.001) in adolescent girls with self-harming behaviors.
Conclusion: Based on the research findings, dialectical behavior therapy, by using emotion perception techniques, positive self-talk about emotions, physical vulnerability reduction skills, decision-making skills, and distress tolerance skills, can be used as an effective method to reduce rumination and feelings of inferiority in adolescent girls with a history of self-harming behaviors
