Background and Aim: Pain is a sensory experience that persists and leads to emotional distress for many individuals. The interaction between the physical and emotional perception of pain is a complex, multidimensional phenomenon. The present study aimed to examine the validity and reliability of the Bodily and Emotional Perception of Pain Questionnaire (BEEP-2019) and its relationship with alexithymia in individuals experiencing chronic pain.
Materials and Methods: This psychometric study employed a cross-sectional design. The study population comprised all individuals with chronic pain visiting pain treatment clinics in Shiraz, Iran, in 2023. A purposive sampling method selected 228 participants. The Bodily and Emotional Perception of Pain Questionnaire (BEEP-2019) was translated into Persian. Data were collected using this instrument alongside the Short-Form McGill Pain Questionnaire (SF-MPQ) and the Perth Alexithymia Questionnaire (PAQ). Data analysis was conducted using exploratory factor analysis and Pearson's correlation in SPSS software (version 24).
Results: Exploratory factor analysis with varimax rotation confirmed the three-factor structure of the BEEP-2019 instrument. Cronbach's alpha values exceeding 0.7 supported this structure. The subscales "Emotional Response to Pain" and "Pain Interference with Personal and Social Functioning" demonstrated adequate validity and reliability (α =0.89; α = 0.76). The "Limitations in Daily Activities" subscale showed acceptable reliability (α = 0.67). Convergent validity of the BEEP-2019 with the SF-MPQ was established for the total score (r = 0.59, r² = 0.348) and other subscales (P < 0.01). Furthermore, a significant direct relationship was found between overall pain perception scores and alexithymia (P < 0.01).
Conclusion: The BEEP-2019 questionnaire demonstrates multidimensional assessment capability and favorable psychometric properties, suggesting its superiority over other instruments for evaluating the role of emotions in pain perception. It is recommended for use in research and clinical evaluations involving chronic pain. Future implementation of this scale in therapeutic programs and validation studies across diverse pain populations is advised.
